Referrals explained

Do I need a referral for physio

You want to start physiotherapy, but do you need to see a doctor and get a letter first?

M. Thurairaj, MAHPC-registered physiotherapist review

In Malaysia you can go direct

You do not need a doctor’s referral to see a physiotherapist in Malaysia. A family can contact a physiotherapist directly and arrange an assessment.

This is helpful when travel to a clinic is hard and time matters, such as soon after a stroke or surgery. Waiting for a letter can slow recovery down.

So for many families, the first step can simply be a message to us.

When a doctor should see the person first

Going direct does not mean skipping medical care when it is needed. Some situations call for a doctor before physiotherapy begins.

New and sudden weakness, chest pain, high fever, a fresh injury, or a wound that looks infected all need a doctor or hospital first. A condition that has not been diagnosed yet may also need a review.

We would rather be honest about this than start a plan too early.

How we handle it with you

When you message us, we ask a few simple questions about the person’s history and current state. This helps us judge the safe path.

If home physiotherapy is suitable, we plan the first assessment. If a doctor should see the person first, we say so plainly and explain why.

Either way you get a clear next step, not a runaround.

What a first message to us covers

When a family messages us to start, we keep the first exchange simple. We ask what happened, when it happened, and how the person is right now.

We ask whether a doctor or hospital has already seen them, and what was advised. If there is a discharge letter, a photo of it helps.

From those answers we can usually tell whether to plan a visit or suggest a doctor first. There is no need to prepare anything formal.

The aim is to save you a wasted trip either way. A short conversation often settles the referral question quickly.

If we ever need more before deciding, we will ask plainly rather than guess.

Ask if you can start

If you are unsure whether you can start physiotherapy directly or should see a doctor first, tell us the person's situation and we will point you to the safer step.

Common questions

If we have a referral letter already, is that useful?

Yes, very. A referral or discharge letter tells us the diagnosis, the surgery, and any precautions. It helps us plan safely from the first visit. If you do not have one, we can still start after a careful assessment and a few questions.

Will physiotherapy work with our doctor or specialist?

Where it helps, we keep our work in line with what the doctor has advised, especially any movement limits after surgery. If something we see suggests the doctor should review the person, we will tell you so you can follow up.

Can we start physiotherapy while waiting for a specialist appointment?

Sometimes, and sometimes not. It depends on the condition and whether it is safe to move before the specialist has seen the person. Tell us the situation and we will give you an honest view on whether to start now or wait.
